His grandfather was Joseph Patrick Kennedy, Sr.
His uncle (Joe Sr.'s son) was Joseph Patrick Kennedy, Jr.
So shouldn't he be Joseph Patrick Kennedy III?

Because when you are the Second or II, you are named for your grandfather, not your father. To be the Third, you, your father and the grandfather all have to have the same name. In order to be the Third, your father, not your uncle, has to be Junior.
In my case, it would be:
Chauncey Q. Buttercup or Chauncey Q. Buttercup, Sr.
Chauncey Q. Buttercup, Jr.
Chauncey Q. Buttercup, III
Chauncey Q. Buttercup
Elmor Y. Buttercup
Chauncey Q. Buttercup, II
